General Information


The Florida Voter Registration Application is a standard statewide form and can be used by all permanent residents of the state seeking to register or update their voter registration records. 

In order to register to vote one must:

 

  • Be a citizen of the United States
  • Be a legal resident of Florida
  • Be 18 years old (may pre-register at 16 but will not become an eligible voter or receive voter information card until 18th birthday)
  • Not have been adjudicated mentally incapacitated with respect to voting, or if so, competency has been restored
  • Not be a convicted felon, or if so, voting rights have been restored
  • Not claim the right to vote in any other county or state

 

The registration form is available both in English and Spanish.

The registration form may be photocopied. 

An applicant who fails to designate party affiliation will be registered without party affiliation. 

If the information provided on the form is not true, the applicant may be convicted of a felony of the third degree and fined up to $5,000 and/or imprisoned for up to five years. If an applicant has been convicted of a felony or has been declared mentally incompetent with respect to voting by a court of law, and is unsure whether his/her civil rights have been restored, he/she may call the Supervisor of Elections office at 407.742.6000. An applicant should fill out his own registration form and must sign it. However, you may help an applicant to fill out a form if he/she needs assistance.

The form does not require the applicant's signature to be witnessed.  In some cases a copy of the applicant's identification may be required.  It is suggested the instructions on the application be reviewed before completing the voter registration application.  Each applicant should be encouraged to print legibly and be reminded to sign and date the application form. It is illegal to influence or attempt to influence a person in the free exercise of his/her right to register to vote.
